medical insurance outside Europe

I need to sort some medical / emergency cover for a 3 week trip riding my own bike outside Europe. I’ve been down a few dead ends with quotes, most places say riding is excluded or they’ll only cover hire bikes or bikes up to 125cc. Any recommendations?

We use them for travel insurance and it covered me in India.
Annual cover seems good value so used them for years.
