I think I can see an issue if not the cause, ball bearing are not suitable for anything but small radial loads. You should be using taper roller bearings that transmit the axial and radial loads that you find in sidecar applications.

It's bigger than it looks but most complain that bikes are to big these days and this is probably built with that in mind.

Yes, I'd like to know as well. What brought you to change the bearings. Were they notched and if so was it straight ahead or a bit to the left.

That looks like a work hardened fracture to me, bin it the material around the crack will be brittle. What are the fork guides like, grab the bottom of the fork and see if it moves in relation to top tube. There are a lot of non design stresses on conventional forks with a sidecar bolted on, I'm sur...
